Do You Offer a Maintenance Program?
For patients who have already completed their initial year or for existing patients who would like to continue with the practice, we recommend one of the existing plans as a maintenance program. The Whole Blue Program is considered the minimum maintenance program recommended.
If you need additional care or wish to customize your program, please discuss with your provider.
What is the Cancellation Policy?
New Members:
If you cancel your membership within 72 hours of signing the membership agreement and before you schedule your first appointment, you will receive a full refund.
If you have signed the membership agreement and you cancel your membership more than 72 hours after signing your membership agreement or after scheduling your first appointment, but greater than 7 days from your first scheduled appointment, you will receive a full refund minus a $150.00 onboarding fee.
If you have signed the membership agreement and scheduled your first appointment, and cancel your membership within 7 days of your scheduled appointment, you will be issued a full refund minus $300.00 ($150.00 deposit for your first reserved appointment plus the $150.00 onboarding fee).
Existing Members:
If you choose to cancel your membership you are required to give us 30 days written notice.
When you cancel early, prior to the end of your annual term, you will be charged for the services you have already used, according to the following rates:
New Patient Onboarding Fee $150.00
Clinician New Patient Visit (90 minutes): $600.00
Clinician Follow Up Visit (45 minutes): $300.00
Health Coach Follow Up Visit (60 minutes): $150.00
Health Coach Follow Up Visit (30 minutes): $ 75.00
Cancellation Fee $150.00
Provider Time to Respond to Messages, $25.00 per 5 minutes
For example, if you signed up for the Whole Blue Plan (a $1499.88 a year plan), and you give notice at 3 months, and the services you used were
new patient onboarding, new patient clinician visit, 1 health coaching session, 1 follow-up clinician visit, and cancellation:
New Patient Onboarding Fee $150.00
New Patient Clinician Visit (90 minutes): $600.00
Health Coach Visit (60 minutes): $150.00
Clinician Follow Up Visit (45 minutes): $300.00
Cancellation Fee $150.00
The charges for services provided would total $1350.00
If the total amount you paid was $499.96 (if you were paying monthly @$124.99/month x 4 months),
the remaining balance you would owe ($1350.00 – $499.96) would be $850.04
This is an example. The final amount you owe will depend on the amount you have already paid minus the amount of services you have already used, minus the new patient onboarding fee and the early cancellation fee. Please contact us for details and we will calculate the exact amount of your refund.
What Happens if I Need More Exam-Based Follow Up Visits than My Membership Provides?
For clients that enroll in a Whole Blue, Whole Gold, or Whole Platinum Membership and need additional exam-based follow up visits and/or communication support, you have several options:

You can upgrade your membership, for example from Whole Blue to Whole Gold, or Whole Gold to Whole Platinum, however you may be asked to pay off the balance of your initial plan

You can remain on your existing membership in which case you will be charged at a default rate of $75.00 per 15 minutes of provider time for additional exam-based follow up visits

You can wait until your next annual cycle begins to re-initiate your care; your electronic health record, as well as your Fullscript account will remain accessible to you
What Happens if I Exceed the Exam-Based Provider Messaging Support that My Membership Provides?
For clients that enroll in a Whole Blue, Whole Gold, or Whole Platinum Membership and need additional exam-based provider messaging support, you have several options:

You can continue to message your provider and will be charged for additional provider time for messaging support $25.00/ 5 minutes provider time, with a $25.00 minimum

You can elect to request a provider visit to address follow up questions, which can be more cost effective
